On average 120 older people in Sheffield are admitted or re-admitted to Hospital every month due to:-
ISOLATION - LONELINESS - LACK OF SUPPORT
What can be done about it?
Agewell supports over 200 groups in all areas and communities in Sheffield. Our groups are available to anyone aged 50 plus and have proved to be successful in preventing and combatting isolation and loneliness.
Older isolated people can refer themselves to Agewell. They can also be referred to join an Agewell Group and/or a Lunch Club by family, friends, neighbours and care providers. Furthermore, GP's, District Nurses or other Community Health Professionals, can also refer older clients as part of their individual care package.
Each referred client is contacted by Agewell staff and where required home visits are carried out by staff and volunteers all of whom have been checked and cleared by the Criminal Records Bureux. Clients are provided with the opportunity to join their nearest group or lunch club and when necessary are accompanied by staff during their initial attendance at the group. When required staff utilise the opportunity to sign-post clients to other agencies and organisations that can provide help and assistance. This may include Pensions advice, housing and health services, social services, Home Care and voluntary agencies such as Sheffield Carer's Centre.
